  • Supported development and implementation of a PrEP pilot in two provinces, and built capacity of health professionals to deliver PrEP services.
  • Supported finalization of the National AIDS Spending Assessment 2016-2020, which revealed declining resources for the national HIV response, in particular for prevention services targeting key populations.
  • Over US$ 6 million were mobilized from the Global Fund for the 2023-2025 period for HIV prevention for key populations; the Joint Team assisted technically the process.
  • As part of the Global Partnership against stigma and discrimination and through broad consultation, Ecuador identified health, justice and community as the priority settings to focus on for the next years.
Joint Programme Results

The Joint Team in Ecuador focussed its support in scaling up access to HIV services among young people and key populations, particularly by reinforcing community-based HIV responses. For instance, community surveillance networks were strengthened in the four provinces of the Northern Border with Colombia—Carchi, Esmeraldas, Imbabura and Sucumbíos while a youth-led working group was created in Quito-Pichincha, establishing coordination spaces for a comprehensive and intersectoral HIV response. As a result, a total of 2332 people, including people living with HIV were sensitized on accessing information, HIV and health services, social protection programmes and human rights. A community school for the training of trainers on health, including HIV and human rights was also established to educate young people residing in border and cross-border areas. The first cohort of 20 trained young people replicated local workshops in urban and hard-to-reach areas and reached 6625 young people with information on HIV prevention, sexual and reproductive health and human rights (UNFPA). Additionally, 150 young people and migrants in the Northern Boarder and Quito-Pichincha received information and referrals to HIV and other health services through community-based workshops, thanks to a collaboration with MoVIHlízate, the youth-led movement of young people living with or affected by HIV (UNHCR, UNFPA).

In 2022, Quito and Guayaquil cities were offering community-led pre-exposure prophylaxis (PrEP) services and 200 people from key populations accessed these services. A total of 96 healthcare providers from Guayaquil, Machala, Manta and Quito were also trained on the delivery of PrEP service and progress has been made to rollout pilot PrEP services in Machala and Manta cities (WHO). Additionally, 15 representatives of the Ministry of Health and civil society organizations improved their knowledge of PrEP, HIV self-testing, community-led Monkey Pox prevention and strategic information (WHO, UNAIDS Secretariat).

Access to sexual and reproductive health services increased among refugees and migrants through information and training sessions on HIV, sexually transmitted infections, menstruation, family planning and contraception methods, reaching 4174 forcibly displaced people. A total of 2286 hygiene kits and 8844 condoms were also distributed and 249 persons were tested for HIV (UNHCR). A community-based intervention led by Corporación Kimirina, a nongovernmental organization, further facilitated access to prevention and diagnosis services to 6432 migrants from key populations (IOM).

To eliminate mother-to-child transmission of HIV and syphilis in Ecuador, four priority hospitals in Guayaquil received technical support, including capacity building to improve HIV case detection and retention in treatment, which benefited more than 10 000 pregnant women and women of childbearing age in 2022. In addition, three healthcare institutions in Conocoto, Guayaquil and Manta were certified as mother- and child-friendly facilities under the Amigos de la Madre y del Niño health facility certification programme led by the Ministry of Public Health, with technical support from the Joint Team. To further build on these efforts, a total of 1000 pregnant women were trained to educate their communities on the prevention of vertical transmission of HIV and syphilis (UNICEF).
